The British Columbia Ministry of Justice has released its findings on what caused the death of "Glee" star Cory Monteith last July.

The coroner's report issued Wednesday stated that the 31-year-old actor died of   "[i]ntravenous heroin use in combination with ingestion of alcohol." The death was ruled accidental.

The report also notes that a spoon with drug residue and a used hypodermic needle were found by emergency responders, along with two empty champagne bottles and several glasses.

Monteith's body was discovered at the Fairmount Pacific Rim hotel in downtown Vancouver on July 13.
