Cory Monteith, who plays Finn Hudson on FOX's "Glee," has checked himself into a rehab facility.

The 30-year-old actor's rep issued a statement saying that Monteith has "voluntarily admitted himself to a treatment facility for substance addiction.

"He graciously asks for your respect and privacy as he takes the necessary steps towards recovery."

Monteith is no stranger to addiction struggles; he first received treatment when he was 19 and has spoken openly about his issues with addiction, telling Parade magazine in 2011 that he was "lucky to be alive."

Monteith is dating his "Glee" co-star Lea Michele, who told People "I love and support Cory and will stand by him through this," adding, "I am grateful and proud he made this decision."
